How do i upgrade from ios 5.0.1 to 5.1 or above?

I cannot find clear instructions for upgrading my iphone 4 to ios 5.1 or above.
Also I am wondering if its worth it to upgrade to ios 6
A while ago it seemed i would loose much more than i would gain.
