Mediata Survey is a desktop application for Mac and Windows to create web surveys.
The application employs an easy to use GUI that allows the user to formulate multiple pages with multiple questions per page. Various question formats are accommodated including: yes/no, multiple choice, or write in. Conditional branching or skip logic can be easily implemented, and the application automatically creates the PHP scripts that can be uploaded to the user's web server. Questions can be created in an exam format, or with different answers assigned different point values. The application generates a flow chart providing a quick overview of all logic. Surveys can be configured as public or by invitation only.
