I am looking for instructions on how to unlock an iPhone 4s running iOS 6.1.3 with baseband version 3.4.03 so that it can be used with different carriers.

The most effective way to unlock an iPhone 4s is through an official IMEI unlock provided by the original carrier. You should contact the carrier the phone is currently locked to and request an unlock. If the device is out of contract, they usually perform this service for free. Once they approve the unlock, insert a SIM card from a different carrier and connect the iPhone to iTunes to complete the process. Software-based unlocks like ultrasn0w do not work on the iPhone 4s, and hardware interposer SIMs are often unreliable and not recommended.
