I want to downgrade my iPhone 5 from iOS 9 to iOS 8.4.1. I am using Verizon and see two different IPSW files available. Which one should I download, the iPhone5,1 (GSM) or iPhone5,2 (GSM+CDMA) version?

Since you are with Verizon, your iPhone 5 is likely the CDMA+GSM model (iPhone5,2). You can verify this by checking the model number printed on the back of your device. Model A1429 corresponds to the iPhone 5 (GSM+CDMA) or iPhone5,2 firmware, while model A1428 is the GSM-only version. To proceed, identify your model number and download the matching IPSW file from this page: www.iclarified.com/750/where-to-download-iphone-firmware-files-from. Once downloaded, connect your iPhone to your computer and put it into DFU mode. Use iTunes or Finder to restore the device using the file you selected. Note that while Apple usually stops signing older firmware, iOS 8.4.1 is often used in specific downgrade paths for the iPhone 5. If a standard restore does not work, you may need to use specific tools designed for the 8.4.1 OTA downgrade method.
